These forms are provided in PDF and/or Word format and are located under the form name. To use a PDF form, you will need Adobe Acrobat 5.0 or later versions. Adobe Acrobat Reader is a free product available from www.adobe.com. Once you open the PDF form, type the text in the highlighted fields. If the fields are not highlighted, select "highlight fields" in the upper right-hand corner of the document. If you only have Adobe Acrobat Reader installed on your computer you will not be able to save the additions made to the Adobe PDF form.
